Register With The Practice?

The practice serves the area of Gorseinon and its boundaries as outlined on the map within the website.

To register as a patient, please bring in your medical card or ask at reception for a registration form.

It is our policy to invite all new patients for a health check.

Please note that we can only accept patients who live in our practice area.

How Do I....
Change My Details?

It is important that you notify us immediately of any amendments to your personal details, ie change of surname, address or telephone number.
Failure to do so may result in administrative errors which may in turn affect patient care.
